* Deliver personalized counseling services to TB patients, caregivers, and service providers through inbound and outbound interactions, driving treatment adherence and positive health outcomes. * Architect referral coordination strategies with DOTS centers, ensuring seamless transitions and comprehensive support for patients. * Manage accurate and confidential documentation of calls in digital systems, maintaining up-to-date records and facilitating data-driven decision making. * Drive patient engagement and follow-up, tracking progress and outcomes of referred patients to inform service improvements and optimize care. * Lead patient-centered communication, fostering empathy, trust, and open dialogue to empower patients and caregivers in their healthcare journeys.
