Key Responsibilities
* Drive community-level health initiatives by conducting regular household visits to identify and support pregnant women, newborns, and children under five years of age.
* Deliver basic health education on Maternal, Newborn, and Child Health (MNCH) services, including safe pregnancy, newborn care, nutrition, hygiene, and family planning, to empower community members.
* Facilitate access to essential health services by supporting registration and follow-up of pregnant women for antenatal care, delivery, and postnatal care, and promoting institutional delivery.
* Champion early detection and intervention by monitoring growth and development of children, providing nutrition counseling, and identifying danger signs in pregnant women, mothers, and children.
* Mobilize community members to participate in immunization sessions, outreach clinics, and other health initiatives, fostering a culture of health and wellness.
* Lead community outreach efforts by organizing meetings, awareness sessions, and behavior change communication activities to promote healthy practices and disease prevention.
* Manage data collection and reporting by maintaining household registers, beneficiary lists, and submitting accurate and timely data to supervisors.
* Foster partnerships with local health facilities, community leaders, and volunteers to ensure coordinated and effective health service delivery.
* Support the development of vulnerable groups, including women, children, elderly persons, and persons with disabilities, by facilitating their access to essential health services.
* Engage in continuous learning and professional development by participating in regular training, review meetings, and capacity-building sessions to enhance skills and knowledge.
* Provide emergency response support and participate in health camps and outreach services as required, ensuring timely and effective response to community health needs.
* Promote respectful, culturally sensitive, and confidential engagement with community members, upholding the principles of dignity and inclusivity.
* Architect community-level initiatives to promote hygiene, sanitation, and disease prevention practices, contributing to a healthier and more resilient community.
