Key Responsibilities
* Deliver high-quality, community-based palliative nursing care through home visits, ensuring comprehensive support to patients and their families.
* Conduct thorough assessments of patients' physical, psychological, social, and spiritual needs to inform personalized care plans.
* Administer medications and manage symptoms such as pain, breathlessness, and nausea, ensuring optimal patient comfort and well-being.
* Perform a range of basic nursing procedures, including wound care, catheter care, and feeding support, to address patients' complex needs.
* Provide emotional and psychosocial support to patients and family members, promoting a holistic approach to care.
* Empower family caregivers through education and training on home-based patient care, enabling them to provide effective support.
* Mentor and supervise Palliative Care Assistants (PCAs), fostering a culture of excellence and continuous learning.
* Maintain accurate and up-to-date patient records, ensuring seamless communication and coordination with the healthcare team.
* Collaborate with doctors, PCAs, and community volunteers to deliver coordinated and compassionate care.
* Identify patients requiring urgent medical attention and facilitate timely referrals, ensuring prompt access to necessary care.
* Engage in team meetings, case discussions, and training sessions to stay updated on best practices and share expertise.
* Provide end-of-life care and bereavement support to families, offering a supportive presence during difficult times.
* Contribute to community awareness and outreach activities, promoting the importance of palliative care and supporting community engagement.
* Assist in data collection for monitoring and evaluation, informing project development and improvement.
Requirements
Education
- Diploma in Nursing
- Bachelor of Science (BSc) in Nursing
- Certificate course/training in palliative care
Additional
- Previous experience of working in Palliative Care or community-based projects will be given priority
